Other Sleep Concerns

For sleep symptoms that do not fit cleanly into the more common diagnostic boxes — REM paralysis, nightmare disorder, hypnagogic phenomena, exploding head syndrome, and other rare conditions — we evaluate and treat with the same care as for any other patient.

What we mean by "other"

Sleep medicine is a broad field. Beyond the headline conditions — apnea, insomnia, restless legs, narcolepsy, parasomnia — there is a long tail of less common diagnoses, each with its own evaluation and treatment pathway. We see these patients too, and we take the time to get the diagnosis right.

Conditions we evaluate

  • Isolated sleep paralysis — brief inability to move at sleep onset or offset
  • Nightmare disorder — recurrent disturbing dreams that impair daytime function
  • Hypnagogic and hypnopompic hallucinations — vivid sensory experiences at sleep transitions
  • Exploding head syndrome — sudden loud auditory perception at sleep onset
  • Sleep-related rhythmic movement disorder — head banging or body rocking during sleep
  • Confusional arousals — incomplete awakening with disorientation
  • Sleep-related eating disorder — eating during partial arousals from sleep
  • Catathrenia — sleep-related groaning
  • Hypersomnia not otherwise specified — excessive sleep without classic narcolepsy features

Evaluation

  • Detailed clinical and sleep history
  • Polysomnogram with video when behavioral or movement features are prominent
  • Specific testing (MSLT, MWT) when indicated
  • Psychiatry coordination when comorbid mood or anxiety conditions overlap

Treatment

Treatment is condition-specific. Many of these diagnoses respond to a combination of reassurance, sleep hygiene optimization, and targeted pharmacology. Some require ongoing coordination with neurology or psychiatry.
