Geriatric REM Sleep Behavior Disorder

REM sleep behavior disorder in older adults — acting out dreams, vocalizing, sometimes injuring oneself or a bed partner — warrants careful evaluation. It can be an early biomarker of Parkinson''s disease, Lewy body dementia, or related neurodegenerative conditions.

What RBD looks like

In healthy adults, the body is paralyzed during REM sleep — so we dream without physically acting on the dreams. In REM sleep behavior disorder, that paralysis is absent or incomplete, and patients act out their dream content. The behaviors range from sleep talking to vigorous movement to serious injury.

Why it matters beyond the bedroom

For older adults, RBD is now recognized as a leading prodromal indicator of synucleinopathies — Parkinsons disease, Lewy body dementia, multiple system atrophy. The diagnosis carries practical implications for monitoring, family discussion, and care planning. It does not, however, guarantee neurodegenerative onset — and we are careful in how we communicate this.

Our evaluation

  • Sleep medicine and neurological consultation
  • Polysomnogram with video and limb EMG to confirm RBD diagnostically
  • Cognitive screening when appropriate
  • Coordination with neurology for ongoing prodromal monitoring when indicated
  • Safety counseling for patient and bed partner

Treatment

  • Safety modifications — bed rails, padded surroundings, separate sleeping arrangements when needed
  • Melatonin — often first-line, well-tolerated, modest efficacy
  • Clonazepam — effective but used cautiously in older adults
  • Coordinated care — with neurology and primary physician as the case evolves
